Every request for records must be served from our secure Release Of Information Portal. Our office does not accept any type of process serving, including, fax, email, and personal service. Please upload your request for records from our Upload Portal Module
It's our policy to request an Initial Processing Fee of $15 for every request for records (Medical, Billing, X-Rays, Therapy) as each file item is retrieved and processed separately. This fee is required in advance to start the retrieval process and is payable to Records Source our Exchange of Health Information service.
This Initial Processing Fee is only to process your request for records, and it's not for the cost of the records reproduction. This Initial Processing Fee is a non-refundable fee to cover the clerical costs incurred for the following:
- HIPAA Disclosure: HIPAA requires that all Covered Entities must have a system in place to track the release of our patients Protected Health Information (PHI).
- Records Request Process: It takes valuable clerical time to process, search, verify and data entry patient's record requests. Without this fee, your request for records might not be processed.
- NO Records Found/ CNR: If after a complete search of records (digital, paper, storage), no records are found, your request will be closed with a Certificate of No Records and your Initial Processing Fee will cover the cost of all the clerical time spent in complying with your request.
- Records Found/ Production Cost: If after a complete search of records (digital, paper, storage) records are found, a Final Invoice for the reproduction cost, the certification process, and the digital distribution of the records, will be submitted to your office for approval. The final records reproduction fee must be payable to our medical facility, for our valuable time in making records available. Please follow the instruction on the Final Invoice.
All requests are subject to fifteen 15 business days turnaround, from the day of processed. Please do not call us during this period of time. We will keep you posted on any further updates by fax and email.

Accounts that are inactive, will move to inactive status after 180 days and will not be accessible to the end user. To resolve this, please send us an email to support@calsportsandspine.com.
- Your request needs to be serve to Records Source —with an initial processing fee of $15. This fee must be payable to Records Source. Without this fee, your request for records will not be processed.
- Our doctors provide treatment to patients at different medical facilities. Treating doctor's name and address must be included in your request.
- Your request must be directed to the medical facility, Records Source is not a treating medical facility.
- We required individual requests for Medical, Physical Therapy, Billing and X-Rays records —as each file item is retrieved and processed separately.
There is no need to spend the time, money on gas, or postage fees to serve us your request for records. E-serve us, it's faster, easier and convenient. An Affidavit of Electronic Service will be provided by fax and email as soon as you paid the fifteen $15 Initial Processing Fee.

You can always check the status of your request from our Online Status Portal, all you need is your unique Reference Number and the patient's Date of Birth.
As soon as your request is complete, we will send you an email notification. You can download files as many times as you need for only five days, from the notification notice.
After this time frame, your file is no longer available. If for whatever reason we re-upload a file, we will charge you an upload fee.
You will contact Records Source for any questions regarding records. As soon as your request for records has been served properly, we will contact you by email.

You can always check the status of your request from our Online Status Portal, all you need is your unique Reference Number and the patient's Date of Birth.
You can easily pay for records online. Most of our medical providers accept digital payments through the PayPal Payment Gateway, and you don't need a PayPal account to pay for records.

If you have made payment for records by check, please email a copy of the cleared check (front and back). As soon as we verify your payment information, records will be made available to download.
Avoid processing delays, do not serve the medical provider, they will not process your request. If you choose to serve the medical provider, it will take us more days to start the process of your request.

We do not serve Private Health Information by email. When you receive an email from Reports Source is only a notification, the report is not attached. If you need the report, you must login to your secure account and download.
From your account, you can view all your New, Downloaded and Offline reports. You can download files as many times as you need for Sixty 60 Days and request any report at any time, when it has been placed offline.
Our interface is very easy to use, when you receive an email file notification:
- Login to your account at https://www.calfiles.com/login
- Download new reports, under the "Reports = NEW" section.
- Download new notices, under the "Notices = NEW" section.
- You can download any file as many times as you need for 60 Days.
Perhaps, the report is not ready and has not been upload it to your account. Please do the following:
- Fill out the report request form.
- Be specific on the type of report needed, under "Special Instructions".
- We will contact the facility on your behalf and try to get the report.
- If the report is ready, we will e-serve it to your account.
- A notification will be send to your email on file.

There are NO fees to access or download medical reports. We do not charge you to join, or to share your reports in a secure, electronic format with your opposite counsel.
We always try to serve applicant and defense attorneys. However, if your firm is not listed on our proof of service list, please e-serve us a copy of your Notice Of Representation and those reports will be made available to your firm.
If you wish not to be notify when a new report becomes available, please go to your Account "Profile" and disable "eMail Notifications". Please, keep in mind that you will be responsible to check your account periodically.
If you receive a notification and the report is no longer needed, or you're not the associate handling the case, please disregard the notification. You can also deactivate the feature of "eMail Notifications" from your account profile, this way you will stop receiving notifications when a new report becomes available. Please, keep in mind that you will be responsible to check your account periodically.
"Claims administrators' offices and representatives' offices may designate first class mail, electronic mail or fax as their preferred method of service for receiving documents from the district office and the appeals board. The designated method of service shall be the same for all active cases for that claims administrator's office or representative's office".
(c) The primary treating physician, or a physician designated by the primary treating physician, shall make reports to the claims administrator as required in this section. A primary treating physician has fulfilled his or her reporting duties under this section by sending one copy of a required report to the claims administrator. A claims administrator may designate any person or entity to be the recipient of its copy of the required report. (d) The primary treating physician shall render opinions on all medical issues necessary to determine the employee's eligibility for compensation in the manner prescribed in subdivisions (e), (f) and (g) of this section. The primary treating physician may transmit reports to the claims administrator by mail or FAX or by any other means satisfactory to the claims administrator, including electronic transmission.
